This form serves as your official request for support from the Center for Open and Digital Teaching and Learning (CODLT) – Educational Media Production (EMP) team for the production and post-production of educational media such as webinars, hybrid events, and video/audio materials. It is intended for UPOU faculty, staff, and units requiring assistance in content recording, editing, uploading, or live streaming.

To help us serve you better, please provide complete and accurate details about your event. Before submitting, please take a moment to read the Terms, Conditions, and Data Use Policy in line with the Data Privacy Act of 2012.

Before accomplishing this form, please ensure that a formal request has been sent via email to the CODTL. Kindly note that only approved requests will be processed to support proper coordination, manpower availability, and efficient use of technical resources. Pre and Post Production

NOTE: Please request the venue for one full day for setup and tech run, separate from the actual event.

Please indicate when the material is needed if editing is required.

This information is necessary to confirm that event coverage aligns with activities intended for academic use and can be produced as Open Educational Resources (OER).

A signed Data Privacy Form must be accomplished by the resource speaker(s) and submitted prior to event coverage.

The University of the Philippines Open University (UPOU) collects and processes personal information through this form solely for the purpose of evaluating, processing, and monitoring event coverage requests and related services. Personal information will be handled in accordance with the Data Privacy Act of 2012 and related issuances of the National Privacy Commission and will be accessed only by authorized UPOU personnel.

By accomplishing the form, you, as the data subject, hereby give your consent and authorization to the processing of the personal information you have provided for the above-stated purposes. This consent/authorization remains valid and subsisting for a limited period consistent with the purposes above or until otherwise revoked or cancelled in writing.
